To ensure you are ready for your first visit with us at Excel Physical Therapy and Sports Rehabilitation, we have compiled a list of things to expect after scheduling with us.
You will need to come in 15 minutes prior to your scheduled appointment time to fill out a few forms. If you prefer, you may download these forms here and bring them with you or you may email them to firstname.lastname@example.org.
If you do choose to fill out the paperwork at home we do ask that you still come in a few minutes prior to your appointment.
If your insurance company requires a prescription, referral, written order, and/or prior authorization, please bring this with you if we do not already have one for you on file.
Your insurance card(s), L&I claim number, or your MVA claim number, and photo ID
Any co-pay if applicable
All appointments run 45 minutes, the number of follow up visits will depend on the nature of your condition, severity and complexity.
If we are assessing a lower extremity, bring in a pair of shorts
If we are assessing your gait or doing a running assessment bring in a pair of shorts along with your running shoes.
If we are assessing an upper extremity, upper back or neck bring in a tank top or sports bra (for women).
If you have any questions, please feel free to call us. We are here to help. We do require a 24 hour notice if you are unable to make your appointment. This will allow other patients the opportunity to be seen.

**We are closed daily from 01:00pm-02:00pm for lunch. Outside of business hours feel free to leave a message and we will contact you as soon as possible.
